The Concierge Program

A team of health professionals to expertly & consistently guide your medical journey.

We Believe

The patient should be at the helm, actively developing their personal narrative with the support of Clinical & Lifestyle Guides, who help navigate the complexities of the medical system, investigate and identify next steps, translate progress, and confidently communicate their questions and desires.

Who We Serve

Our Concierge Program is designed to support patients who value the help of a trusted partner–whether it’s to help closely review their regular health concerns or assist in facing a serious chronic illness or complex medical issue.

What We Do

We help our clients anticipate their health needs on a consistent basis, providing high-touch care. Our Guides help you to review and understand your medical care, including your portal. We walk with you, side-by-side, as your Story Stewards, throughout your journey.

We also consult with clients on an ongoing basis giving them a more holistic view of their health and empowering them to deepen the mind-body connection, increase their resiliency, and develop their personal narrative.

“Thank you for being there for my dad & me, when we needed it most”

— Lisa D. (Minnesota), Concierge Program Client

Our Guides take advantage of critical hinge points in their clients’ journey to educate and help them successfully navigate health transitions.

“The team helped me better understand my role as a patient. I advocated for my story and my relationship with my care team improved. I trust my physician more now because he knows what I expect from my care.”
